To work in the Hospital social work team to assist with discharge planning and longer term work on from discharges.
The social worker should have experience of hospital social work, ability to discharge patients in a safe and timely way back to their homes. To carry out Care Act assessments, MCA's Safeguarding, Working with people who have No recourse to public funds ( NRPFs) ability to organise placement s and extra care sheltered placements.
work in a client centred way with a focus on strengths perspective towards independence as far as can be achieved in partnership with our clients.
Ability to work in a Multi Disciplinary way and promote integrated care.

- Dipsw or Degree in Social work and registration with HCPC
