To work with an Adult Social Care Review Team which is Countywide and also works with service users placed out of county.
To undertake current assessment of needs as part of the review, thinking about the right size of provision, use of equipment, assistive technology etc.
To ensure accurate recording and evidence of need against Care Act 2014 eligibility.
To undertake standalone OT reviews.
To undertake holistic needs led assessments.
To ensure accurate recording and clear SMART support planning.
To ensure safeguarding processes are followed as necessary.
Specifications
- Degree or diploma of the College of Occupational Therapy and current registration with the Health and Care Professionals Council (HCPC)
