Specifications - HCPC registered, previous OT experience essential.
A qualified band 6 Occupational Therapist (or local authority equivalent) with experience in moving and handling assessments, major and minor adaptations, DFG processes, Blue Badge assessments and reviews
Will be based in a local authority OT team and working within patients own homes they will complete moving and handling assessments (including a focus on single handed care) and associated care plans, occupational therapy assessments focusing on major and minor adaptations including DFG process all with the aim of maximising independence and reducing dependence on services.
The OT service sits within a wider front door social work team and a wider social work service that is integrated with continuing health care nurses so effective communication is essential.
The ability to manage a complex case load and deal with difficult family dynamics
Be able to work both independently and as an effective team member alongside other professionals within health and social care services.
The applicant will be required to work 37 hours per week 5 days cover Monday to Friday.
