To hold a caseload of people with complex mental health needs and undertake comprehensive needs led assessments
To contribute to the provision of effective after care services, including those required under Section 117 and the Care Programme Approach.
Participate in multi-disciplinary systems as appropriate, including joint screening and assessment as required
The successful candidate to be familiar with Health and Safety procedures and good practice and implement these in all aspects of working practice and promoting them in the team and workplace.
The successful candidate to be familiar with the Comments, Compliments and Complaints Policy and Procedure. Also good practice relating to this and implement this in all aspects of working practice. This includes encouraging service users, relatives and carers to have a voice in the provision of services and acting as an advocate as appropriate.
The successful candidate to ensure that confidential information, which is made available to the post holder in order that they are able to undertake their duties or is otherwise known to them, is kept confidential and secure at all times.
Specifications
- Qualified Social Worker (registered with HCPC)
- Experience of applying the Care Act (2014), Mental Capacity Act (2005) and managing adult safeguarding enquires.
