Qualified Social Worker required for the North Discharge Planning team. One to be based at Hinchingbrooke Hospital and one to be based at Peterborough Hospital. Cover is required from time to time.
Strong hospital discharge experience (at least 2 years within hospital setting)
Good discharge to assess experience within 72 hour framework
Experienced in CHC / use of DSTs in caseload
Can commit to at least 6 months, ideally 1 year contract
Monday – Friday working only
Ideally experienced in using Mosaic
Specifications
- Qualified Social Worker and registered with the Health and Care Professions Council (HCPC).
- 2 years+ PQE
Start Date
Asap – looking at a start of mid-September
