As a service manager, be accessible to patients, carers/ families and other community and acute teams.
The lead for ensuring that assessment, delivery, and evaluation of care provided to patients is of the highest quality and appropriate to meet their clinical needs.
To support the reduction of avoidable emergency admissions to the local acute providers through the development of community and primary care integrated services.
Ensure the patient experience is of the highest quality, inspiring patient, public and professional confidence.
Specifications
- Experience of a nursing back ground
- Registered nurse with a current NMC registration
