Hertfordshire County Council are looking for a Social Worker to join their team in the Lister Hospital, Stevenage. To assess the needs of people in the area who require social care support andadvice. To provide services that meet the outcomes identified in the assessment and reviewservices to ensure they continue to meet the identified outcomes. To safeguard vulnerable adultsAll work to be carried out with users of our service, their families and carers within theframework of the seven social care outcomes:Improved health and emotional wellbeing. Improved quality of lifeMaking a positive contributionExercising choice and control Maintaining personal dignity Economic wellbeing and freedom and freedom from discrimination.
Specifications
- Experience working with Adults.
- Experience of a Hospital environment
- Qualified Social Worker and registered with the Health and Care Professions Council (HCPC)
- 2 years PQE
